Casualties: Unknown Medical Guide

How to read your health panel and treat the injuries that actually kill you, based on the game's First Aid course and the official patch notes. Medicine recipes are at the end.

Pre-release page. Launches on Steam November 9, 2026 at 3pm CET (14:00 UTC). Data here comes from the Steam page, the demo and official developer posts, and each page says which version it reflects. We'll update it for the full release.

Version: demo 7.0.1 First Aid course and patch notes up to 7.0. The full game makes some injuries less punishing (September 2026 update), so some numbers may change at 1.0. This is game advice, not real medical advice.

Reading the health panel

Open it with R. The First Aid course walks through each readout:

  • Brain integrity: when it reaches zero, you die. Almost everything else on the panel exists to protect it.
  • Blood volume: treat about 2.5 L as the lethal limit.
  • Blood pressure: normal is about 120/80. Heart rate, blood volume and whether your vessels are constricted or dilated all affect it.
  • Heart rate and ECG: the ECG shows your rhythm, and it turns distinctly wavy as fibrillation gets worse.
  • Respiratory rate and SpO2: breathing too little lowers your blood oxygen.
  • Immunity: how well you fight off infection.
  • Absorbed radiation: anything above zero means trouble.
  • Consciousness, pain, rate of bleeding, stamina, and a per-limb panel.

You don't need to memorize this. Your moodles summarize most of it. See the full conditions list.

Bleeding

Bleeding is the most common injury. It lowers your blood volume, and without enough flowing, oxygenated blood your brain dies. Wounds can also get infected. Wrap a dressing around the wound in the health panel. Always carry some.

  • Each bleed severity has its own moodle, from Minor bleeding to Catastrophic bleeding (6.0).
  • Internal bleeding can come from low abdomen muscle health (7.0), and its moodle also scales with severity.
  • Hemothorax lowers your maximum oxygen (7.0).
  • Procoagulant thickens your blood (6.0), and clotting mush was buffed in 7.0. Don't overdo it: see blood viscosity below.
  • Blood loss lowers blood pressure, and that's what sets off the heart problems below.

Heart: fibrillation and cardiac arrest

When blood pressure drops, your heart speeds up to compensate. At very high heart rates or very low pressure, fibrillation starts: the rhythm gets more erratic until the heart stops. The First Aid course says it kills in about two minutes. The first sign is an Arrhythmia moodle.

  • AED (automated external defibrillator): use it on your chest and follow the steps. It resets fibrillation to zero and stabilizes you for a while.
  • Manual defibrillator: it can fail. Set the power level to match how far fibrillation has progressed, low if it just started and high if you're seconds from arrest. The closer the match, the better the chance it works. Use the ECG and the power estimate to judge. Intelligence also affects how well you use one.
  • Since 6.1, fibrillation only rises quickly above 280 bpm, and amiodarone brings it down faster.
  • Cardiac arrest: an autopump can keep you conscious while your heart is stopped. Epinephrine, found in combat pens, restarts the heart. Defibrillators can also work but take longer, and since 6.1 they don't always fix asystole.

High blood pressure and stroke

Hypertension is usually more manageable than low pressure, but if it gets severe you can have a stroke. Keep blood pressure below 180/120. During a hypertensive crisis the stroke chance is 2% per second (6.1). Strokes can be switched off in run settings. Opioids cause vasodilation (6.1), and the First Aid course lists vessel constriction and dilation as one of the things that set your blood pressure.

Blood viscosity, embolism and breathing

  • Blood viscosity (how thick your blood is) isn't shown on the panel. Both extremes cause problems, but very thick blood is the more dangerous one.
  • Very thick blood can cause a pulmonary embolism, which damages your thorax. Streptokinase breaks the clot down.
  • If thorax muscle health gets too low, you stop breathing. A bruise kit on the thorax restores it.
  • Low breathing lowers SpO2, which leads to unconsciousness, fibrillation and death.

Infection and sepsis

  • Apply antiseptic to the infected site every few minutes. The arrow on the infection icon shows whether it's shrinking or growing.
  • Infections are lethal if you ignore them. Being well fed and in good shape slows them down.
  • Sepsis, Severe sepsis and Septic shock are separate, more serious conditions (see conditions). Since 6.1, sepsis affects blood pressure much less.
  • You can make a sterilized dressing from a dressing and antiseptic.

Dislocations, fractures and limbs

  • Dislocations usually come from bad falls and hurt a lot when you use the limb. Fix them by hand: build up speed with your hand and click hard over the joint.
  • Fractures need time or tools. A splint speeds up healing a lot, and a dressing on the same limb helps a little.
  • A limb stops working when its muscle health gets close to zero. A useless hand can't hold things.
  • Full game: lost limbs can be replaced with prostheses in 3 tiers, and maintaining them works differently from healing (September 2026 update).

Pain and opiates

  • Severe pain immobilizes you, worsens your mood quickly and can make you pass out from shock.
  • Opiates reduce pain across your whole body quickly. Overdose is easy, and withdrawal can follow. Naloxone and naltrexone help with opiate problems.
  • The opiate chain in demo 7.0: opium, then morphine, then fentanyl, each made from the previous one (see recipes below).

Venom and radiation

  • Venom was added in 6.0. Cave ticks inject 75% less per bite since 7.0. Antivenom is craftable.
  • Radiation: any absorbed radiation is bad. Anti-rad treats it, but you can overdose on it (6.0). The Ionized layer modifier means nuclear waste, but it also spawns some anti-rad (6.0).

FAQ

How much blood can you lose in Casualties: Unknown?

The First Aid course says to treat about 2.5 litres of blood volume as the lethal limit.

What is a normal blood pressure in Casualties: Unknown?

Around 120/80, according to the First Aid course. Try not to let it go above 180/120, because severe hypertension can cause a stroke.

How do you fix fibrillation?

Use a defibrillator on your chest. An AED resets fibrillation to zero. With a manual defibrillator, matching its power level to how far fibrillation has progressed raises its chance of working. Untreated fibrillation kills in about two minutes.

How do you restart your heart?

Inject epinephrine, which you find in combat pens. Defibrillators can also work but take longer, and since demo 6.1 they no longer always fix asystole.
